As a medical doctor, I assisted to a surgery to repair the patient's hole in the abdomen. Is it unethical to give a medication that will put the patient to sleep during a surgery that will save the patient's life? Or give pain medication to a patient that is in extreme physical suffering? I am refering to the precept about intoxication and drugs. It is very important to me to keep the five precepts.

No it does not break the precept because:

  1. You are not the one being injected
  2. You do not have an intension to intoxicate the person. 
  3. Going to sleep is not intoxication.
